As a new patient, there is information we will need. We have put our new patient forms online so you can fill them out before your visit. This can include your medical history, billing information, health insurance and other basic information. You can submit this online – we use a digital, paperless system for patient records that is easier for you to access and less waste for the environment.
Our office accepts most major insurance plans to keep your dental costs as low as possible. We also have specials for new patients and a discount plan available. It is important to us to help our patients manage their dental care costs to make a healthy, beautiful smile affordable for everyone.
